Configuring RDS Certificates on Server 2012 R2 RDS

I have just setup a Windows Server 2012 R2 RDS and now I'm not sure about the certificate. I think this is the last part of my configuration. I have already activated the server and installed the license CALs. In RD Licensing Manager, the server shows activated and I have installed 100 per User CAL. 

One question - in the deployment overview - Edit deployment properties - RD Licensing - I have set "Per User" Mode.

When I was installing the licensing, there was a mention to select "Per Device". Which is correct?

Should I not use self assigned certificates? It looks like I will need to export from the RD Gateway and import the certificate to all clients machines. Is this correct?  Do I need to purchase a certificate from a CA authority even though my users are only going to have internal access? If so, what should the name of the certificate be? For instance, my server is MYSERVER.mydomain.com.

Do I use that name or something like portal.mydomain.com?
